Sciatica Chiropractor in San Diego: Chiropractic Care for Sciatic Nerve Pain & Relief

Sciatica can make everyday life uncomfortable. Sharp, shooting pain may travel from your lower back through your buttock and down your leg. You may experience burning, tingling, numbness, or weakness that makes sitting, walking, driving, exercising, or sleeping difficult.

At Greater Life Wellness Center in San Diego, Dr. Henry Wong, DC provides personalized chiropractic care for patients experiencing sciatica, lower-back pain, and related nerve symptoms.

Serving San Diego since 2001 Palmer West graduate Near the Sports Arena & I-8
The Short Answer

Can a Chiropractor Help With Sciatica?

Yes. Chiropractic care can help some patients with sciatica, particularly when symptoms are associated with mechanical problems involving the lower back, spinal joints, or surrounding neuro-musculoskeletal structures.

Sciatica often develops when a nerve root in the lower spine becomes irritated or compressed. Depending on the underlying condition, chiropractic adjustments, corrective exercises, and movement-based care may help reduce pain and improve function.

At Greater Life Wellness Center, we evaluate spinal mobility, neurological findings, posture, and movement patterns to develop a corrective care plan tailored to the patient's needs.

Chiropractic care offers a conservative, hands-on approach that may help selected patients manage sciatica without immediately pursuing more invasive treatment.

Understanding Sciatica

What Is Sciatica?

Sciatica describes pain and other symptoms associated with irritation of the sciatic nerve or the nerve roots that form it.

The sciatic nerve originates from nerves in the lower spine and travels through the buttock into the leg. When these nerves become irritated, symptoms may extend well beyond the lower back.

Sciatica most often affects one side of the body, although symptoms can occasionally occur on both sides. Some patients experience significant leg pain with very little back discomfort.

Sciatica is a symptom pattern, not a single diagnosis.

Understanding which structures are involved helps guide appropriate chiropractic care. For information about other spinal and musculoskeletal conditions, visit our Symptoms & Services page.

Common symptoms include:

  • Sharp, shooting pain traveling down one leg
  • Burning or electric-like sensations
  • Tingling or pins-and-needles feelings
  • Numbness in the buttock, leg, or foot
  • Muscle weakness
  • Pain that worsens with certain positions
  • Difficulty sitting, standing, or walking comfortably
Causes

What Causes Sciatica?

Several conditions can irritate the nerves that contribute to sciatic symptoms.

Disc problems

Herniated or Bulging Discs

The discs between your vertebrae act as cushions that help distribute forces throughout the spine.

When a lumbar disc bulges or herniates, displaced disc material or associated inflammation can irritate a nearby nerve root. This may cause pain traveling from the lower back into the buttock, thigh, calf, or foot.

Not every disc bulge causes symptoms, and the location of an abnormality must be considered alongside the examination findings.

Learn more about chiropractic care for bulged and herniated discs.

Narrowing

Spinal Stenosis

Spinal stenosis occurs when the spaces surrounding spinal nerves become narrowed. In the lower back, this may contribute to leg pain, numbness, tingling, or weakness.

Some patients notice that standing or walking aggravates symptoms, while sitting or bending forward provides relief.

The appropriate treatment depends on the severity of narrowing and neurological involvement.

Age-related

Degenerative Disc and Joint Changes

Age-related changes in spinal discs and joints may contribute to nerve irritation when they narrow the spaces through which nerves travel.

However, degeneration on an imaging study does not necessarily explain a patient's symptoms. A chiropractic evaluation considers the complete clinical picture rather than relying on imaging findings alone.

Read more about disc degeneration and chiropractic care.

Arthritis

Bone Spurs and Arthritic Changes

Arthritic changes can sometimes produce bony growths called osteophytes.

When these growths reduce the space available for a spinal nerve, they may contribute to radiating pain or neurological symptoms.

Outside the spine

Muscular and Soft-Tissue Problems

Not every painful sensation traveling into the leg originates from a spinal disc.

Muscles and soft tissues around the hips, pelvis, and buttocks can produce symptoms that resemble sciatica. In some cases, the sciatic nerve itself may become irritated outside the spine.

Identifying the difference is important because treatment approaches vary.

Spinal Mechanics

Why Spinal Function Matters With Sciatica

Your lower back does more than support your body weight. It allows you to bend, twist, walk, lift, and move while protecting the nerves that travel from the spinal cord into the lower extremities.

When spinal joints become painful or restricted, surrounding muscles may tighten and movement patterns can change. These mechanical problems do not explain every case of sciatica, but they may contribute to discomfort or reduced function.

As a chiropractor, Dr. Wong evaluates how the lumbar spine, pelvis, hips, and surrounding muscles work together.

  • Lumbar spine
  • Pelvis
  • Hips
  • Surrounding muscles

Improving spinal mobility and addressing relevant mechanical restrictions can be an important part of conservative care for selected patients with sciatic symptoms.

The goal is to improve function while respecting the condition of the affected nerves and tissues.

Our Approach

How Chiropractic Care Can Help Sciatica

Chiropractic care focuses on the relationship between spinal and joint function, movement, and neuro-musculoskeletal health.

At Greater Life, our process is based on examination findings rather than applying the same adjustment or exercise program to every patient.

1

Specific Chiropractic Adjustments

Chiropractic adjustments use controlled, targeted forces to improve movement in restricted joints.

For certain patients with sciatica and associated mechanical lower-back pain, spinal manipulation may help reduce discomfort and improve mobility.

Dr. Wong selects techniques according to the patient's condition, comfort, neurological findings, and physical needs.

2

Corrective Exercises

Improving spinal and joint mobility is only part of the process. Corrective exercises can help strengthen muscles that support the spine, improve movement control, and increase tolerance for everyday activities.

Depending on your condition, recommendations may focus on:

  • Trunk and core strength
  • Hip mobility
  • Spinal stability
  • Balance and coordination
  • Comfortable movement patterns
  • Gradual return to activity
3

Soft-Tissue and Mobility Care

Muscle guarding and stiffness can accompany lower-back and leg pain.

When appropriate, soft-tissue techniques and gentle mobility work may help improve comfort and movement.

Exercises are selected carefully because aggressive stretching or certain positions can aggravate nerve-related symptoms.

4

Posture and Daily Movement

Your work habits, driving position, exercise routine, and daily activities may influence your symptoms.

For example, someone whose leg pain worsens after prolonged sitting may benefit from modifying sitting duration, workstation setup, and movement habits.

Learn more about why your back hurts after sitting.

5

Individualized Progression

As your body begins to improve and heal, your recommendations may change.

The focus may shift from reducing discomfort toward improving mobility, strengthening supporting muscles, and restoring normal activities.

Progress is evaluated through changes in pain, neurological symptoms, movement, and daily function.

Adjusting Techniques

Greater Life Uses Several Approaches

  • Diversified
  • Palmer package
  • Specific prone
  • Drop-table
  • Upper cervical
  • Instrument adjusting
  • Other techniques

The objective is to restore appropriate joint movement and improve function, not to claim that every adjustment directly removes pressure from a spinal nerve.

Evaluation

What We Look for When Evaluating a Patient With Sciatica

When someone comes into our office with pain traveling down the leg, we want to understand exactly how the symptoms began and what may be irritating the nerve.

Not every patient who describes sciatica has the same condition. One person may have symptoms associated with a lumbar disc problem. Another may have spinal stenosis, mechanical back pain with referred symptoms, or irritation involving tissues around the hip and buttock.

We usually begin by asking where the pain starts, how far it travels, and which movements or positions make it better or worse.

Our examination may include:

Spinal mobility

How well the lower back moves and whether certain movements reproduce symptoms.

Orthopedic testing

Clinical tests that help identify possible sources of back and leg pain.

Neurological assessment

Muscle strength, reflexes, and sensation when nerve involvement is suspected.

Posture and movement

How the patient sits, stands, walks, and performs everyday activities.

Previous injuries and imaging

Whether prior conditions or diagnostic findings are relevant to the current symptoms.

We also want to know how sciatica is affecting the patient's life.

  • Can they sit through a workday?
  • Drive comfortably?
  • Sleep?
  • Walk for exercise?
  • Lift their children?
  • Return to the gym?

These functional goals help shape an individualized recovery plan.

Our approach is to understand the likely source of the symptoms, identify the mechanical problems that can be addressed through chiropractic care, and help patients regain comfortable movement.

Telling Them Apart

Sciatica vs. Other Sources of Pain

Sciatica vs. Ordinary Lower-Back Pain

Although sciatica and lower-back pain frequently occur together, they are not the same condition.

Mechanical lower-back pain may remain localized to the lumbar spine or surrounding muscles. Sciatica generally involves symptoms extending into the buttock or leg along a nerve-related pathway.

A person may experience:

  • Lower-back pain without leg symptoms
  • Sciatica with little or no back pain
  • Both lower-back and radiating leg pain

This distinction matters because treatment should reflect the likely source of the symptoms.

For a broader explanation, visit our San Diego back-pain guide.

Sciatica vs. Piriformis Syndrome

Piriformis syndrome and other forms of deep gluteal nerve irritation can produce buttock pain and symptoms resembling sciatica.

The piriformis is a muscle located deep in the buttock. In some individuals, structures in this region may irritate the sciatic nerve.

Because symptoms can overlap with lumbar nerve-root problems, a careful examination is important.

Pain traveling into the leg does not automatically mean that a disc is herniated.

Conservative Care

Can Sciatica Improve Without Surgery?

Yes. Many people with sciatica improve through conservative management and do not require surgery.

Depending on the cause and severity, conservative care may involve activity modification, appropriate exercises, chiropractic treatment, medication prescribed or recommended by a medical professional, or other non-surgical approaches.

Surgery may be considered when symptoms persist despite appropriate care or when certain serious neurological problems are present.

At Greater Life, chiropractic care offers a conservative option for selected patients whose examination findings support this approach.

Our focus is on improving function, managing mechanical pain, and monitoring progress.

Timeline

How Long Does Sciatica Take to Improve?

Recovery varies from person to person. Some episodes improve over several weeks, while others persist longer, particularly when symptoms involve significant nerve irritation or longstanding spinal changes.

Factors that influence recovery include:

  • The underlying cause
  • Severity and duration of symptoms
  • Neurological involvement
  • Previous injuries
  • Activity and work demands
  • Overall health
  • Response to treatment

We monitor how patients respond to care and whether their symptoms and functional abilities are improving. If symptoms worsen or progress does not occur as expected, the diagnosis and treatment approach should be reassessed.

Everyday Questions

Sciatica and Your Daily Routine

Can Sitting Make Sciatica Worse?

Yes. Prolonged sitting can aggravate symptoms in some patients, particularly when certain spinal positions increase discomfort. This is common among people who spend extended periods:

  • Working at a desk
  • Driving
  • Studying
  • Traveling
  • Working from home

Changing positions periodically, adjusting workstation ergonomics, and incorporating appropriate movement may help reduce aggravation. However, the best sitting position or exercise depends on the underlying condition.

Read our guide to back pain after sitting.

Is Walking or Exercise Good for Sciatica?

For many patients, staying appropriately active is an important part of recovery. Walking and gentle exercise may help maintain mobility and physical conditioning without placing excessive demands on the lower back.

However, certain movements can aggravate nerve symptoms. A patient with disc-related sciatica may respond differently to walking or bending than someone with spinal stenosis.

The goal is to find movements that are comfortable and gradually restore activity without repeatedly provoking radiating pain or neurological symptoms.

If an exercise increases weakness, numbness, or significant shooting pain, stop and seek professional guidance.

Can Stretching or Massage Help Sciatica?

Stretching and soft-tissue care may help when muscular tension contributes to discomfort. However, sciatica is not always caused by tight muscles.

Aggressive hamstring stretching, for example, may aggravate symptoms in some patients with nerve irritation.

This is why Greater Life emphasizes individualized recommendations rather than giving every patient the same stretches. Chiropractic adjustments, corrective exercises, and mobility recommendations are selected according to the patient's examination findings.

Sciatica During Pregnancy

Pregnancy changes the physical demands placed on the spine, pelvis, and surrounding muscles. Changes in posture, weight distribution, and joint loading may contribute to lower-back, buttock, or leg discomfort.

Not all pregnancy-related leg pain is true sciatica, and symptoms should be evaluated appropriately.

Chiropractic care using pregnancy-appropriate techniques may help some patients with mechanical back or pelvic discomfort. Learn more about chiropractic techniques for pregnancy-related back pain.

Your First Visit

What to Expect During Your Sciatica Chiropractic Visit

At Greater Life Wellness Center, we believe that effective chiropractic care begins with understanding the patient.

1

Consultation

We review your symptoms, health history, previous injuries, daily activities, and treatment goals, then ask about the location of your pain, whether it travels down the leg, and whether numbness, tingling, or weakness is present.

2

Chiropractic Examination

May include spinal range of motion, posture and movement assessment, orthopedic testing, muscle strength, reflexes and sensation, joint mobility, and functional limitations.

Signs It's Time

When Should You See a Chiropractor for Sciatica?

Consider scheduling a chiropractic evaluation when:

  • Lower-back or buttock pain travels into your leg
  • Sitting or driving aggravates your symptoms
  • Recurring sciatica interferes with work or exercise
  • You experience persistent stiffness or restricted movement
  • Pain disrupts sleep or normal activities
  • Home care has not provided adequate relief
  • You want a conservative evaluation of your back and leg symptoms

A chiropractic examination can help identify mechanical and neurological findings relevant to your condition and determine appropriate next steps.

Warning Signs

When Sciatica Requires Emergency Medical Attention

Most cases of sciatica are not medical emergencies, but certain symptoms can indicate serious nerve compression.

Seek emergency medical care for:

  • New loss of bladder or bowel control
  • Numbness around the groin or saddle area
  • Rapidly progressing neurological weakness
  • Severe symptoms involving both legs, especially with neurological changes

New or worsening leg or foot weakness should be evaluated promptly.

These symptoms should not be managed solely through routine chiropractic treatment.

FAQ

Frequently Asked Questions About Sciatica and Chiropractic Care

Can a chiropractor help relieve sciatica?

Yes. Chiropractic care may help selected patients with sciatica, particularly when mechanical spinal problems contribute to their symptoms. Your care may include adjustments, corrective exercises, and movement recommendations.

What is the fastest way to relieve sciatica?

There is no single fastest treatment for every patient. The best approach depends on the cause, severity, and neurological findings. An examination helps determine which conservative forms of care are the most appropriate.

Can chiropractic adjustments help a herniated disc?

Spinal adjustments can help some patients with disc-related lower-back pain or sciatica, but they are not appropriate for every disc condition. Depending on the severity of a herniated disc, an MRI or other imaging may be required to further diagnose the issue.

Is chiropractic care better than medication for sciatica?

Chiropractic care and medication serve different purposes. Chiropractic care generally focuses on neuro-musculoskeletal function and mechanical pain, while medication may help manage symptoms. The appropriate choice depends on the diagnosis and individual needs and goals.

Can sciatica cause numbness in the foot?

Yes. Irritation of lumbar nerve roots may cause tingling or numbness extending into the calf, ankle, or foot. Persistent or worsening symptoms should be evaluated.

Can sciatica cause leg weakness?

Yes. Nerve-root involvement can affect muscle strength. New or worsening weakness requires prompt medical assessment.

Does sciatica always require surgery?

No. Many patients improve with conservative management. Surgery is generally considered for selected cases involving serious neurological problems or symptoms that remain disabling despite appropriate treatment.

Is walking good for sciatica?

Walking is often helpful when tolerated, but the appropriate duration depends on the condition. Patients whose symptoms worsen significantly with walking should have their symptoms evaluated.

Can sitting all day aggravate sciatica?

Yes. Prolonged sitting can worsen symptoms in some patients. Changing positions, adjusting workstation ergonomics, and incorporating appropriate movement may help.

How long does sciatica last?

Some episodes improve over several weeks, while others persist longer. Recovery depends on the underlying condition, severity, neurological involvement, and response to the proper care.

Can chiropractic care help recurring sciatica?

Chiropractic care may help selected patients manage recurring mechanical back pain and associated sciatic symptoms. A comprehensive evaluation can identify relevant mobility limitations, activity triggers, and other contributing factors.

Do I need an MRI for sciatica?

Not always. MRI may be recommended when significant or progressive neurological symptoms are present, serious pathology is suspected, or imaging is needed to guide further treatment decisions.

Should I see a chiropractor or a medical doctor for sciatica?

A chiropractor can evaluate many neuro-musculoskeletal causes of back and leg pain. Severe or progressive neurological symptoms, suspected serious conditions, or problems requiring medication, surgery, or specialist treatment warrant medical evaluation.

Can I exercise while receiving chiropractic care for sciatica?

Often, yes. Appropriate movement and strengthening can complement chiropractic care. Exercises should be selected according to your symptoms and should not cause progressive neurological problems.

Does Greater Life treat patients from Ocean Beach, Pacific Beach, and Mission Valley?

Yes. Greater Life Wellness Center welcomes patients from these communities and throughout San Diego at its Point Loma/Midway office.
